A Living At Home consultation and Zoom assessment by Tori Goldhammer, MS, OTR/L, ATP, CAPS, ECHM, CEAC, CLCP proved invaluable in one of my recent Life Care Plans. Tori provided recommendations substantiated by using objective assessment tools. Her recommendations addressed functional deficits, safety, and quality of life issues.
